Short-Term Care Management: Tailored care management services to address main focus areas. Great for someone needing short-term assistance from 30 to 90 days.

Personal Preparedness Package: Billed at an annual rate this package is strictly for those that do not currently need care management services but want peace of mind for the future. It helps the individual to organize and gather imperative documents, vital personal contacts, important instructions for emergency personnel, and other information in one central location. Giving the entire care team peace of mind. A Care manager checks in with the client and family on a quarterly basis via telephone and completes two home-visits per year. The client will also remain in the Hope Aging Care monitoring system allowing a care manager to help quickly in a time of need.
